Many thanks to Ted Lightbown for giving us permission to publish his book.  It now acts as a memorial to his knowledge and research skills and, as you will gather from reading the book, his dry sense of humour.  As with everything in the historical/archaeological area there will be new revelations.  Any important clarifications will be linked in as appropriate.
